Characterization of Strokes Following Gore Thoracic Branch Endografting Requiring Zone 2 Landing

چکیده
The Gore thoracic branched endograft is the only currently approved device for treatment of aortic pathology requiring landing in zone 2. While overall stroke rate pivotal trial has previously been reported, a more granular description not yet published. This descriptive series detailing neurologic sequelae from trial, out to 12 months. All patients underwent Modified Rankin Scale assessment at screening, discharge, 1-, 6-, and 12-month intervals. A disabling was defined as occurring within 30-days index procedure, combined with ≥2 an increase baseline least one grade. Descriptive statistics important associations pathology, native aorta vs Dacron graft, intraoperative ballooning were noted. Further granularity regard distribution type are also where available. total 238 included trial. breakdown treated pathologies were: aneurysm, 84; dissection, 132; traumatic transection, 9; other, 13. 1-year 4.6%, highest aneurysm cohort (aneurysm, 7.1%; 3.0%; 0%; 7.7%). frequency strokes 4.8%; 1.5%; other lesion, 7.7%. Forty percent occurred 30 days both hemorrhagic well nonhemorrhagic events. left carotid (n = 1), posterior circulation 2), unknown 3). after primarily (7/9), 3), right 2). Five suffering had component surgical whereas remainder landed aorta. Eleven 17 tri-lobe arch. majority experienced partial or complete recovery (12/17). prosthesis associated comparable 30-day rates similar undergoing 2 debranching procedures. Stroke patients, no transection cohort. Perioperative nonhemorrhagic, those perioperative period ischemic nature. distributions covered multiple territories, suggesting embolic etiology. These data better define risks this technology arch disease.

Endografting in the aortic arch - does the proximal landing zone influence outcome?
OBJECTIVES To analyse early and midterm results of thoracic aortic endografting (TEVAR) in the aortic arch. METHODS Between January 1997 and February 2009 178 patients received TEVAR in the aortic arch at our institution. This population was subdivided into four groups according to the proximal landing zone (LZ) classification in the aortic arch by Ishimaru et al. and a retrospective analysis...

BACKGROUND Conventional surgical repair of thoracic aortic dissections is a challenge due to mortality and morbidity risks. OBJECTIVES We analyzed our experience in hybrid aortic arch repair for complex dissections of the aortic arch. METHODS Between 2009 and 2013, 18 patients (the mean age of 67 ± 8 years-old) underwent hybrid aortic arch repair. The procedural strategy was determined on t...
متن کاملConformable Gore® TAG® Thoracic Endoprosthesis for the treatment of thoracic aortic aneurysms
Although relatively uncommon, descending thoracic aortic aneurysms are lethal, with a natural history of progressive dilatation, rupture and death. The traditional treatment of these aneurysms involves open thoracotomy and replacement of the aneurysmal aorta with a prosthetic graft. Although successful and durable, this treatment is potentially associated with significant morbidity and mortalit...
